Short description

This tool is the discharge plan that the patient receives at discharge from hospital in wards benefiting from clinical pharmacists in our institution. The clinical pharmacists also include this chart in the discharge letter which is sent to the general practitioner and kept in the hospitalization report. The plan is completed in a language adapted to the patient but data in the lower boxes are more detailed in the letter and the hospitalization report for the doctors.

It is used since 2004 in the Cliniques universitaires Saint-Luc Brussels, but has been recently improved.

Registered in the computerized chart of the patient;
quality of the information depends on the pharmacist completing the data.
