This is a simple tool to record all patient information during the drug history.
Applicability (setting e.g. inpatient care, outpatient care, long term care etc.)
The tool is used for inpatient care.

Ideally each patient entering the hospital must receive a drug history of a pharmacist, but often there is lack of time and so it is sometimes difficult. The tool is easy and patients understand it well. Sometimes we have to take information from elsewhere (family, nursing home …)
