Short description

This tool was developed for German hospitals which are implementing Medication Reconciliation within the High 5s initiative (see It contains information and advice on conducting the Best Possible Medication History within the Medication Reconciliation process, such as an interview guide and examples of documentation forms. An English version does not exist yet but is planned.
The guide was developed using content modified from the Medication Reconciliation Acute Care Getting Started Kit developed by ISMP Canada (see as well as materials from national initiatives. The project teams in the German High 5s hospitals reviewed the draft and their feedback was incorporated into the final version.

Target audience

Health care professionals who collect the Best Possible Medication History such as pharmacists, physicians, nurses, pharmacy technicians and those responsible for training them and implementing the process.

The guide was disseminated to those hospitals implementing Medication Reconciliation within the High 5s initiative (7 hospitals as of May 2013). It is yet unclear whether these hospitals are applying the tool in routine care. This will be evaluated in 2014.
