Aim and the benefit of the Patient Safety Practice

The aim of the project is to prevent inappropriate medication/medication errors at a plastic surgery ward in Denmark (Aarhus University Hospital, Denmark) and to get an overview an assessment of the departments adverse events. The benefit of the project is to use a systematic method to medication errors, ensure patient safety and quality of care at ward level. There has been developed a generic guide for doing a medical tracer which can be used at different wards (not only plastic surgery). It is important to involve the management of the ward in the planning of the tracing.

Description of the Patient Safety Practice

The patient safety practice refers to the set-up of a medical tracer at the plastic surgery ward. A tracer is a systematic review of the treatment and care provided at the ward. In this project, the focus was on medication. The practice involves a systematic review of the adverse events reported during the hospital stay (medical errors) with the aim of assessing the risk for new events and the prevention of those that already occurred. On the basis of the tracing the following interventions were systematic implemented at the ward: patient identification (when and how); medical labeling; the patients storage/safe-keeping of medicine in dispensing packages. The planning of a medical tracing should be done by quality and patient safety coordinator of the ward/hospital or together with another ward. It is important that the results of the tracing are presented for the management in order to make sure that following initiatives are implemented.
