The patient safety program scheduled to Consorci Hospitalari de Vic (CHV)established in the systematic work of the Quality program, and will be encouraged to five basic principles (5):

• Leadership within the organization: Incorporation of strategic objectives on safety.

• Consider human limitations in the design process: design of tasks taking into account safety, avoid relying on memory and vigilance, simplification of processes.

• Promote effective teamwork: forming teams and involve patients in the care process.

• Anticipate what is unexpected proactive analytical procedure for action when an error occurs and promoting computerized HC (information at the point of care).

• Create a learning environment: simulations, promoting declaration errors, ensuring no punitive attitude, work culture ....
Description of the Patient Safety Practice
 
During the year 2013 has been set to star theme to work the issue of hand hygiene, ensuring the implementation of the WHO protocol, involving professionals and patients, assessing their application.
It also works actively with the issue of feedback with professional results and improvements implemented after analysis of adverse events.
