Transmit to patients a good knowledge to get a correct use of drugs.
Patients receive a programme of training and health education at the beginning and before any change of drug treatment. Oral information is combined with written information, to improve the understanding of the patient about medications he should take (including the importance of compliance).
Description of the Patient Safety Practice
 
The Pharmacy Service is developing a training program for outpatients to improve the use of drugs (including the importance of compliance). The patients have to take drugs and their caregivers or family members have to know how to help them. Usually, these drugs are very expensive and with a high degree of complexity.
This program combines both, oral and written information and the patient may consult any aspect of treatment at home.
We join with the patient to elaborate an individualized drug treatment sheet and to get the best patient´s involvement.

The basic information to be included in the patient leaflet is: drug name (generic and trade), dose (expressed as a pharmaceutical form), frequency and duration (indicating whether or not associated with meals), drug action, major secondary effects and tips on the administration.

The evaluation is done through two indicators:
a) Degree of coverage rate to all outpatients
b) Knowledge of the degree of the patient’s satisfaction
