Elderly patients are highly exposed to Adverse Drug Reactions (ADR)
The aims of our project were: to evaluate the incidence of ADR causing hospitalization and during hospital stay; to identify ADR type and reliable drugs; to alert the health workers on pharmacovigilance and to improve detections and reports of ADR; to increase nurses and physicians multiprofessional integration.
Description of the Patient Safety Practice
 
Data were collected with a simple form compiled by the nurse at admission and discharge and confirmed by the treating clinician. If an ADR is detected, the type of reaction and the suspected drug were noted. Schedules are controlled at discharge and reviewed by the Geriatrician and Hospital Pharmacist, who evaluate the gravity of ADR (by WHO criteria) examinating clinical documents (severe/unexpected ADR were signaled to Pharmacovigilance Deputy). Data were collected in anonymized form evaluated by statistical analysis.
