Based on an online reporting system for critical incidents regularly team meetings over 5 years created a high degree of trust, which allows a very constructive discussion to improve quality and safety.

A password protected file was generated. The password was given to all physicians of the department of anaesthesiology and intensive care medicine. All entries in the database were reviewed in the team. After a starting phase of a few month the discussion became opener. Today all incidents or “near misses” are discussed voluntarily by the involved persons themselves. As the next step also the nursing staff reports in the system.
