The "ApoCIRS" (CIRS = Critical Incident Reporting Sxstem) is a tool where pharmacists can anonymously report critical incidents affecting patients and customers.
Expert analyse and provide advice.
The outcome (i.e. a short descirption of the circumstances and a recommendation) is visible for all colleagues in the intranet. This way all pharmacists are alert and further incidents which might put patients on risk can be avoided.

Desciption of the Workflow:
A Pharmacist happens to make a mistake or nearly made a mistake. After logging in the intranet section of our homepage he anonymously makes a short description of what has happened.
A clearing point gets this information and removes all personal data and identifiability from the text. If additional information is necessary the clearing point can contact the reporter (pharmacist).
Then the facts of the case are forwarded to a committee of experts. They analyse and assess the case and make a recommendation for the best behaviour/reaction in this situation.
The description of the case and the recommendation is visible for all pharmacists in the intranet section.
Thereby, we raise the awareness of all colleagues and hope to avoid similar incidents in the future.
