Description of the Patient Safety Practice

The practice was started in 2012. Population: elderly patients (age =65 years) with poly pathologies and poly prescriptions hospitalized at the SOC of General Medicine, that presented an ADR upon admission and/or during hospitalization. A database was developed with the following information: • admission reference number • name • surname • date of birth • sex • age • hospitalization period • admission diagnosis • home drug therapy • hospital drug therapy • drug interactions • suspect ADRs and onset date • level of ADR severity Training courses for medical staff regarding Pharmacovigilance and the correct use of drugs FINDINGS: Suspect adverse reactions reports previously unreported and found during revision of medical records. The project is still ongoing. The main results achieved so far is the increase in the number of ADR reporting: 312(october 2012- august 2014)- 74 (january 2010- october 2012)
